Managing a kidney condition can feel like a lot. That s why Humana and Strive Health are providing access to kidney care experts. Strive experts work together with your primary care doctor and nephrologist to develop a personalized care plan to keep your kidneys as healthy as possible.

Your Strive Health care team will include nurse practitioners, nurses, dietitians, social workers and others who can visit you in person or connect over the phone, whatever works best for you. They can help you with:

Coordinating with providers

Managing prescriptions

Making healthy food and lifestyle choices

Navigating complications and bumps in the road

Strive Health support can help you live better with a kidney condition

Avoid hospital stays and ER visits

Delay the need for dialysis and transplant
